How to Apply for a Foreclosure Intervention Consultation:
In order to provide you with the best, most relevant information, your foreclosure intervention counselor needs to be familiar with the details of your specific situation, which begins with your completed Foreclosure Intervention Program Application.
An application can be picked up at anytime by stopping by the NHS office
at 125 East Seminary Street in Richland Center, WI.
Our office is located in the lower level of the Historic Park Hotel. Look for the blue awning!

- Open-Print-Complete the Foreclosure Intake Packet with budget sheet
- Additional required documents are listed on the first page of the application.
Copies of the additional required documents should be included with your completed Foreclosure Intervention Program Application. - Please make sure the application is filled out completely and all required documentation is included. If the application is incomplete or any of the required documentation is missing, you will be asked to provide the missing information before we can process your application.
- You can return the application in person or by mail to the NHS office.
You may also fax it to 608-647-8293 or email it as an attachment to info@nhsrcwi.org. - A Foreclosure Intervention Counselor will review your complete application materials and contact you to schedule a foreclosure intervention consultation.
